That's odd... Usually if there is going to be a database error, it will happen when the browser first tries to load the form.

I would suggest double-checking your form in SeaGreen to make sure your field names do not contain any spaces or strange characters and then republish the form to your server.

Just be sure to reset your database schema after republishing.
