I just want to know how can I include my SeaGreen form to my website made in Dreamweaver.

The first step is to leave yourself an empty <TD> or <DIV> tag
on the specific page you wish to merge with (or in your template).

Then, SeaGreen provides you with a built in form merge wizard which allows you to merge your form with the html page which contains the empty tag.

Detailed instructions and screenshots for this functionality can be found in the help files (the PDF can be found in the SeaGreen section of the Iconico website). The bookmark in the PDF file is called Step 1: Form Layout > Form Merge.
